DECLARATION
of 30 July 2018
setting out the models for applications for including professional experience or part thereof
According to § 37 (1) (t) of Act No. 95 / 2004 Coll., on the conditions for obtaining and recognising professional competence and specialised competence for the pursuit of the medical profession of physician, dental practitioner and pharmacist, as amended by Act No. 67 / 2017 Coll., (hereinafter referred to as "the Act '):
Model applications
(1) The model of the application for the inclusion of professional practice or part thereof pursuant to Article 5 (7) of the Act is set out in Annex 1 to this Decree.
(2) The model of the application for the setting-up of professional practice or part thereof pursuant to Section 5 (8) (a) of the Act is set out in Annex 2 to this Decree.
(3) The model of the application for the setting-up of professional practice or part thereof pursuant to Article 5 (8) (b) of the Act is set out in Annex 3 to this Decree.
(4) The model of the application to set aside professional practice or part thereof pursuant to § 5 (9) of the Act is set out in Annex 4 to this Decree.
Efficacy
This decree shall take effect on the 15th day following its publication.
