According to § 25 (a) of Act No. 133 / 2000 Coll., on the registration of residents and birth numbers and on the amendment of certain laws (Act on the registration of residents):
Annex 3 to Decree No. 296 / 2004 Coll., implementing the Act on the Registration of Residents, as amended by Decree No. 299 / 2006 Coll., Decree No. 84 / 2011 Coll. and Decree No. 5 / 2015 Coll., reads:
