DECLARATION
of 20 April 2012
determining the scope of the price entry bill
Pursuant to Section 150 (5) of Act No. 127 / 2005 Coll., on electronic communications and amending certain related laws (Act on electronic communications), as amended by Act No. 468 / 2011 Coll., the Czech Telecommunications Authority provides for the implementation of Section 44 (5) of the Act on electronic communications:
Formats of price entry
The post-clearance price (hereinafter referred to as the "bill ') submitted by the universal service provider shall include the particulars of:
(a) a telephone number;
(b) an indication of the agreed price plan;
(c) the definition of the settlement period on the date of the first and last day of the settlement period;
(d) an indication of the time at which the status of the provider's recording equipment was taken over on the last day of the settlement period, unless the status was taken over at 24.00 hours;
(e) breakdown of individual calls, including calls made within free minutes or other discounts broken down
1. the telephone number (s) to be called, or any other indication clearly identifying the call,
2. the start of the call by indicating the date, hour, minute and second;
3. the length of calls charged in specified unit of measurement,
4. the resolution of the operation to a strong, weak or, where applicable, different for each call, if the universal service provider applies such distinction;
5. the resolution of the call by the network in which the call is terminated;
6. the price charged for a single call in Czech crowns, indicating whether the price is with or without value added tax,
(f) value added tax rate,
(g) the number of available and used spare units, if provided, and information on the transfer of the number of spare units from the previous or subsequent settlement period, if transferred;
(h) the amount charged in Czech crowns for all calls referred to in point (e) in the defined settlement period in total and the sum of the amounts broken down by item of the price list in force by indication of the total amount excluding value added tax, value added tax and the total amount with value added tax;
(i) settlement of advances, overpayments or arrears;
(j) the date of payment of the amount charged;
(k) information on how the claim is to be applied, including where and within which time limits the claim can be applied.
(1) The bill must comply with the requirements of the tax document under the Value Added Tax Act.
(2) If the bill is issued to more than one connection or participant station, it shall include a breakdown of the items referred to in § 1 (a) to (f) for each of these connections or participating stations. The same procedure is followed when multiple telephone numbers are used for one connection.
Efficacy
This Decree shall take effect on 1 June 2012.
