DECLARATION

on the particulars of the application for official authorisation and the application for the termination of official authorisation and the form of verification of the results of the geographical activities for State defence purposes
According to Article 20 (2) of Act No. 200 / 1994 Coll., on Geometry and on the amendment and addition of certain laws related to its implementation:
Forms of application for official authorisation
(1) A written application for official authorization (1) is submitted by a natural person to the Ministry of Defence.
(2) The application contains:
(a) the name and surname of the natural person requesting the granting of an official authorisation (hereinafter referred to as the "applicant"), his / her home number, his / her permanent residence and his / her national identity card number and, if the applicant has a permanent residence outside the territory of the Czech Republic,
(b) the scope of the required official authorisation, (2)
(c) the date and verified3) the applicant's handwritten signature.
(3) The application shall be accompanied by:
(a) an extract from the criminal record not more than six months old, 4)
(b) verified3) a copy (copy) of the diploma and, where appropriate, other evidence of higher education with the award of an academic degree or the status mark "engineer" of the field "geodesy and cartography," "engineering geodesy," "geodesy," "cartography," "surveying," "mining metering," "military engineering geodesy" or "military engineering geodesy,"
(c) details of the applicant's professional experience for at least five years in the activities for which he applies for an official authorisation, including an indication of the period during which the professional experience was carried out and an overview of the geographical activities.
(4) The summary of the geographical activities referred to in paragraph 3 (c) shall state their name and their verification for the individual results of the geographical activities.
Forms of application for an official authorisation
(1) A written request for the termination of an official authority5) is submitted by a natural person to the Ministry of Defence.
(2) The application shall include the name, surname and permanent residence of the officially authorised surveyor, the entry number of his entry in the list kept by the Ministry of Defence (6) and certified (3) of the applicant's own signature.
Form for verifying the results of the agricultural activities
(1) The verification of the professional correctness of the outcome of the surveying activity (7) (hereinafter referred to as "verification of the outcome") is always marked in the lower right-hand corner of the last page of each part constituting the result of the surveying activities. If such part consists of multiple sheets, the sheets shall be firmly joined.
(2) The verification of the result shall be indicated on all its copies which are transmitted to the client.
(3) The handwritten signature, the date of the verification of the result and the registration number of the result validated (8) is attached under the stamp of an officially authorised surveyor.
(4) Verification of the geometrical plan is governed by a separate Regulation. 9)
(5) The size and specimen of the stamp of the officially authorised surveyor are set out in the Annex to this Decree.
Efficacy
This Decree shall take effect on 1 July 1997.

Size and model of the stamp for verification
1. The stamp of an officially authorised surveyor shall have a diameter of 36 mm.
