DECLARATION

amending Decree No. 400 / 2011 Coll., implementing the Civil ID Act and the Travel Document Act, as amended
The Ministry of Interior provides for the Act No. 328 / 1999 Coll., as amended by Act No. 559 / 2004 Coll., Act No. 136 / 2006 Coll., Act No. 227 / 2009 Coll., Act No. 424 / 2010 Coll. and Act No. 318 / 2015 Coll., and pursuant to Article 20 (8) of Act No. 329 / 1999 Coll., on Travel Documents, as amended by Act No. 559 / 2004 Coll., Act No. 136 / 2006 Coll. and Act No. 318 / 2015 Coll.:
Decree No. 400 / 2011 Coll., implementing the Act on Civil Documents and the Act on Travel Documents, as amended by Decree No. 76 / 2014 Coll. and Decree No. 321 / 2015 Coll., is amended as follows:
1. Paragraph 2 shall be deleted, including the title.
2. In Paragraph 4 (1), "Annexes 1 and 2 'is replaced by" Annex 1'.
3. Paragraph 6 (2) is deleted.
Paragraph 3 shall become paragraph 2.
4. Annexes 2 and 7 are deleted;
5. Annex 4, including the title, reads:
"Annex No 4 to Decree No. 400 / 2011 Coll.
Model form of certificate of identity card
"
6. Annex 6, including the title:
"Annex No 6 to Decree No. 400 / 2011 Coll.
Model of the form electronically processed application for the issue of a card with machine readable data and with the contact electronic chip

7. Annex 14, including the title, reads:
"Annex No 14 to Decree No 400 / 2011 Coll.
Model form electronically processed passport application
"
Transitional provision
The forms of the certificate of identity card issued pursuant to Decree No. 400 / 2011 Coll., as effective before the date of entry into force of this Order, may be used until 31 December 2019 at the latest.
Efficacy
This Regulation shall enter into force on 1 July 2018.
