DECLARATION
of 26 February 2004
implementing the Antarctic Act and amending certain laws
According to Section 31 of Act No. 276 / 2003 Coll., on Antarctica and on the amendment of certain laws, hereinafter referred to as "the Act ', the Ministry of the Environment provides for the implementation of Sections 8 (1) (i), 14 (2), 15 (8), 16 (12) and 24 (h) of the Act:
§ 1
Subject matter
The purpose of this decree is to establish:
(a) the list of other substances and preparations hazardous to the environment of Antarctica (Section 2);
(b) details of the notification of the intended activity in Antarctica (§ 3);
(c) the details of the documentation and the opinion on the initial impact assessment (Section 4);
(d) the elements of the documentation, the opinion and the opinion on the universal assessment of the effects (Section 5); and
(e) the model of the observer's special licence (§ 6).
§ 2
List of other substances and preparations hazardous to the Antarctic environment
[K § 8 (1) (i) of the Act]
The list of other substances and preparations which are hazardous to the Antarctic to be authorised for introduction into Antarctica is set out in Annex 1.
§ 3
Forms of notification of the intended activity in Antarctica
(Paragraph 14 (2) of the Law)
Annex 2 sets out the details of the notification of the intended activity in Antarctica, including those already provided for in the Act.
§ 4
Details of documentation and opinions on initial impact assessment
(K 15 (8) of the Act)
(1) The elements of the documentation for initial impact assessment are set out in Annex 3.
(2) The elements of the opinion on the initial assessment of effects (Section 15 (5) of the Act) are set out in Annex 4.
§ 5
Forms of documentation, assessment and opinion on the versatile assessment of effects
(Paragraph 16 (12) of the Law)
(1) The elements of the dossier for the versatile assessment of effects are set out in Annex 5.
(2) The elements of the assessment on the universal assessment of the effects (Section 16 (6) of the Act) are set out in Annex 6.
(3) The elements of the opinion on the universal assessment of the effects (Section 16 (10) of the Act) are set out in Annex 7.
§ 6
Special Observer Card
[K § 24 (h) of the Act]
The model of the observer's special pass (Section 26 (4) of the Act) is set out in Annex 8.
§ 7
Efficacy
This decree shall take effect on the date of entry into force of the law.

Forms of notification of the intended activity in Antarctica
A. IDENTIFICATION OF THE NOTIFICATION
1. in the case of natural persons, the name, surname, date of birth, nationality, telephone and address of the place of permanent residence or, where applicable, foreign residence; the place of business and the identification number, if any, for the commercial natural persons.
2. for legal persons, the name and legal form, registered office, identification number, if assigned, and the identification data referred to in point (a) for natural persons who will carry out activities in Antarctica on behalf of a legal person.
3. in the case of joint notification (Paragraph 14 (1) of the Act), a list of persons who will carry out activities in Antarctica together and the designation of a joint agent for service.
B. DESCRIPTION OF THE ANTARCTIDE PLAN

A. IDENTIFICATION OF THE NOTIFICATION
1. in the case of natural persons, the name, surname, date of birth, nationality, telephone and address of the place of permanent residence or, where applicable, foreign residence; the place of business and the identification number, if any, for the commercial natural persons.
2. for legal persons, the name and legal form, registered office, identification number, if assigned, and the identification data referred to in point (a) for natural persons who will carry out activities in Antarctica on behalf of a legal person.
3. in the case of joint notification, a list of persons who will carry out activities in the Antarctic jointly and the designation of a joint agent for service.
