DECLARATION
of 12 April 2011
on the registration of fuel distributors
The Ministry of Finance provides pursuant to § 11 (2) of Act No. 311 / 2006 Coll., on Fuel and Fuel Gas Station and on the amendment of certain related laws (Fuel Act), as amended by Act No. 91 / 2011 Coll.:
Subject matter
(1) This Decree regulates the model of the notification of the start of the fuel distributor and the model of the fuel distributor registration certificate.
(2) This Decree was notified in accordance with Directive 98 / 34 / EC of the European Parliament and of the Council of 22 June 1998 laying down a procedure for the provision of information in the field of technical standards and regulations and rules and of rules on information society services, as amended by Directive 98 / 48 / EC.
Model of start-up notification and model of registration certificate
(1) The model of the notice of initiation of the activity of a natural person, a fuel distributor, is set out in Annex 1 to this Decree.
(2) The model of the certificate of registration of the natural person, the fuel distributor, is set out in Annex 2 to this Decree.
(3) The model of the notice of initiation of the activity of a legal entity, a fuel distributor, is set out in Annex 3 to this Decree.
(4) The model of the certificate of registration of the legal entity, the fuel distributor, is set out in Annex 4 to this Decree.
Efficacy
This Decree takes effect on 21 April 2011.
