DECLARATION

of 14 February 2005

laying down the model of the questionnaire and model of the military documents

According to Articles 19 (5) and 33 (5) of Act No. 585 / 2004 Coll., on the defence obligation and its provision (the Defence Act), the Ministry of Defence provides:

§ 1

Questionnaire

The model of the questionnaire is set out in Annex 1.

§ 2

Models of military documents

Model military documents are set out in Annexes 2 to 5.

§ 3

They shall be deleted:

1. Decree of the Ministry of Defence No. 260 / 1999 Coll., implementing certain provisions of Act No. 218 / 1999 Coll., on the scope of the defence obligation and on the military administrative offices (Defence Act).

2. Decree No. 16 / 2003 Coll., amending Decree No. 260 / 1999 Coll., implementing certain provisions of Act No. 218 / 1999 Coll., on the scope of the defence obligation and on the military administrative offices (Law No 218 / 1999 Coll.).

§ 4

This decree shall take effect on the day of its publication.
