DECLARATION
of 13 October 2017
on a binding model of a certificate of temporary inability of a candidate to fulfil the obligations of a candidate for employment because of illness or accident and a binding model of a certificate of treatment or examination of a candidate for employment in a medical institution
The Ministry of Labour and Social Affairs provides pursuant to § 21 paragraph 9 of Act No. 435 / 2004 Coll., on Employment, as amended by Act No. 206 / 2017 Coll.:
(1) The binding form of the certificate of temporary inability of the applicant for employment to fulfil the obligations of the candidate for employment on account of sickness or accident is set out in Annex 1 to this Order.
(2) A binding model of the certificate of treatment or examination of the candidate for employment at the health establishment is set out in Annex 2 to this decree.
This decree shall take effect on the day of its publication.
